District Enrollment Forms & Inter District Permits

 

Enrollment Forms Packets

Enrollment application forms for enrolling a student in the following school year will be accepted beginning in early February of each year for La Canada Unified School District residents. Exact dates for when the enrollment window opens each year can be obtained from the district office (818) 952-8395. If you are the parent/guardian of an incoming kindergarten student enrollment forms should be submitted to your neighborhood school's office.  Enrollment forms for grades 1-12 should be submitted to the district office.

Parents of students living outside of district boundaries who wish to apply to La Canada Unified School District for an interdistrict permit should refer to the instructions below.  Enrollment packets for students living outside of the district boundaries are not considered unless the child has been accepted on an inter district permit.

Parents/Guardians are encouraged to begin the enrollment process as soon as possible after the February enrollment window opens.  Although the district tries to place all children at their neighborhood elementary school it is not always possible. Placement for grades K-6 will be made based on where there is space available.

Parents of students who live within the La Canada Unified School District boundaries may apply for their child(ren) to be enrolled in a La Canada public school by downloading, filling out, and submitting the appropriate enrollment forms to the respective elementary school for Kindergarten enrollment and to the district office for grades 1-12 enrollment. Questions concerning Kindergarten enrollment forms should be directed to the respective elementary school office. Questions concerning the enrollment forms for grades 1-12 should be directed to the district office at (818) 952-8395. Different forms are required for different grade levels and all forms within the appropriate packet must be completed and submitted together for an enrollment application package to be considered complete.

The various forms that comprise an enrollment packet can be found in the tables below. Select the table that corresponds to the grade level of the student being enrolled and download a copy of each of the forms in that packet. There are two versions of the forms: a Microsoft Word form template (DOT) and an Adobe Acrobat Reader file (PDF).

Those parents with access to Microsoft Word on their computers should download the Word form templates and fill out the forms using MS Word by clicking on the different form elements and typing in the required information or checking off the appropriate box. (Use the TAB button on the keyboard to advance to the next field on the form.) When each form is complete, use the Save As command to save a copy of the completed form on your computer for your own records and then print out a copy of each completed form, sign it, and submit it as part of the completed enrollment packet to the district office.

Those without access to Microsoft Word should download the Adobe Acrobat version of the forms and print them out using Adobe Acrobat Reader. The forms can then be filled out by hand and submitted together as a packet to the district office.

Inter District Permits

As space allows, La Canada Unified School District considers inter-district permits for students whose parents work within the La Canada Unified School District boundaries.  The following information about how to apply for an inter district permit is also available as a PDF file for you to download and print out here.

Currently, the district is accepting Inter District Permit applications from students whose parents work within the district boundaries of La Canada Unified School District.

The following information is presented in order to help you through the application process.  Please feel free to email La Canada Unified School District Registrar, Chris Roberts, at croberts@lcusd.net or to phone her at (818)952-8395.

  1. Permit applications will be available after April 10th of each year at the district office: 4490 Cornishon Avenue, La Canada, CA   91011. Applications are not currently available on line.  They must be picked up at the district office.  Applications do not take long to fill out and may be completed when you come with your proof of employment on or after the first day that the permit process begins.
  2. Completed applications are accepted starting on April 15th of each year (or the nearest workday after April 15th).
  3. The following documents need to be submitted along with the La Canada Unified School District “REQUEST TO ATTEND SCHOOL OUTSIDE OF DISTRICT OF RESIDENCE FORM”: 
    1. Proof of employment or self employment within the La Canada Unified School District boundaries.  This could include a letter from your employer (on letterhead) stating that you are a permanent employee and a pay stub; a business license; or bills for the business.
    2. Copies of your student’s most recent report card/transcript and any standardized test scores.
  4. A release permit is also required from your district of residence.  Please contact your district of residence for its requirements.  Applications to attend La Canada Unified School District on an inter-district permit are considered when all paperwork is complete and when we have received the approved release permit from your district of residence.
  5. Permits will be approved in accordance with La Canada Unified School District policies and Administrative Regulations (attached to the LCUSD Permit Application).  Since permits are approved based on parent employment, availability of space at the particular grade level needed and the order received, notification of acceptance may not be until August or September. 

The inter-district permit process must be renewed each year by the parent/guardian. This includes the same documentation each year as noted above.  Although rare, students may be withdrawn for any of the following reasons:

  1. Habitual absenteeism, tardies or disruptive behavior.
  2. If the student is suspended for any of the following reasons, the student’s inter-district transfer will be immediately revoked without further notice or hearing:
    1. Serious physical injury to another person;
    2. Possession of a weapon or dangerous object;
    3. Unlawful sale of controlled substances or alcohol or substances believed to be or represented as controlled substances or alcohol;
    4. Robbery or extortion.

If your request for an inter-district permit is granted, Mrs. Roberts will provide the enrollment packet and instructions regarding the enrollment process.
