Technology Equipment Support Request Form

Our NEW form has combined all the information from the old forms and added a few new choices.  Make sure you fill in all the required information.  Select the "Support Catagory" first, then select "Type of Support" and finally the "Brand Name" of the item that needs repair.  The more information you provide the faster the repairs can be made.

  • TV's and VCR's are repaired by an outside service.  Please have the remote
    control available.  If they can't be repaired the replacement will have to
    come out of school site budgets or department budgets.
  • All software installs must have a copy of the software liscense to show that
    software is legal.
  • Toner cartridges and ink cartridges must be purchased out of school site
    budgets or department budgets.
