Step 3 - Documents

Welcome to Resident Enrollment Step 3 - Documents! 
 
This step will list all of the required documents needed to complete the enrollment process - Required documents may vary by grade level. 
 
 
Enrolling for the current 2023-24 School Year 
Please contact our Registrar, Rebecca Augustus via email at [email protected]
to schedule an in person appointment as soon as possible.
 
 
Enrolling for the 2024-25 School Year 
All documents will need to be physically submitted, in person, by one of the three options:
 
1. Document Submissions Days - No Appointment Needed!

    Dates: March 18, March 19 and March 20, 2024
    Time: 9:30AM - 2:00PM & 3:00PM - 5:00PM   
    Location: 4490 Cornishon Ave. La Canada, CA 91011
                      District Office Board Room - Building A
 
2. Walk In Wednesdays - No Appointment Needed!

    Beginning March 27th, walk-ins will be available every Wednesday between
    1:00pm-5:00pm in the District’s Main Office - Building D.
    Wednesday, June 19th our offices will be closed. 
 
3. Appointments - Appointment Required!

    All other dates/times will need to be scheduled in advance through our Registrar.

Please do not use staples or make copies for our records.
All documents will be scanned and immediately returned to you. 
 
 
Below is a printable enrollment checklist for your convenience:
REQUIRED DOCUMENTS
 
ALL GRADE LEVELS : Preschool through 12th
 
ADDITIONAL : TK, Kinder and 1st
Please read the notice attached to the Report of Health!
Oral Health Assessment is due by Kinder and Report of Health (with Eye and Audiology) is due by 1st Grade. These items may be turned in during TK Enrollment if it falls within the 12 month window or 18 months window indicated.
