On the first day of school, students must bring the following documents with them to school:  

• Student Demographic Update Form (signed by parents/guardian)
• Emergency Form
• Notice of Rights of Parent or Guardian
• Internet Use Agreement
• Attendance & Discipline Form

In order to register for grades 9-12, it is mandatory that you download,
fill out and bring the following documents with you to check-in:

• Student Demographic Update Form (signed by parents/guardian)
• Emergency Form
• Notice of Rights of Parent or Guardian
• Internet Use Agreement

9-12 Students will come to the school on their designated day to turn
in their documents, get their textbooks, parking permits, lockers, PE
clothes and class schedules. To ensure that all students have the opportunity to receive their schedules prior to the first day of school, parents and/or family members may, during the student’s scheduled time,
turn in the forms and pick up student schedules in the place of the student.

Additionally, 9-12 schedules will be available for pick up from the Attendance Office on any day after August 20th, 2012 provided the completed forms are submitted. Outstanding obligations such as library
fines or unreturned textbooks must be cleared before schedules are picked up. It is important that each student attempts to go through the registration process in person so that, if necessary, s/he can see
his/her counselor, get a student picture ID, and pick up the textbooks prior to the first class.
