LCUSD STAFF COVID-19 Initial Registration Process

The links below provide access to the registration process for testing at each site. The lab will ask multiple questions for public health data collection. Many questions provide an option of “I prefer not to say.” The site will then collect information on work and insurance status. Insurance will be billed by the lab. If you receive any charges, please contact Mark Evans ([email protected]) and Dolly Leidelmeijer ([email protected]).

We ask that you register and take the test at your primary work location. Check in at the office, and you will be directed to Chris Henry, our school nurse.

TEST RESULTS will be sent to you via text, and if you provide an email, the lab will send the results that way as well. Results will also be sent to the COVID Compliance Team in order to address any positive results. The Team will do COVID-19 Contact Tracing and provide direction regarding quarantine and isolation as needed.
