Caregiver's Authorization

Many adults such as Grandparents, Aunt and Uncles, other relatives or family friends have taken a minor into their home when the natural parent(s) is unable or unwilling to care for the minor. The Caregiver Authorization Affidavit was established to allow such an adult to enroll the minor in school. 

This affidavit does not grant legal custody; Under the Family Code, use of a caregiver authorization affidavit does not affect the rights of the minor’s parents or legal guardian’s regarding the care, custody and control of the minor. The affidavit does not give the caregiver the right to make educational decisions for the minor. 

The enrollment process must be completed by the relative who is assuming caregiver responsibility, not the parent. Please list the legal parent/guardian as the emergency contact if their information is known. 

To preserve the fairness and equity of our enrollment procedures, please note that the district reserves the right to make periodic residence checks throughout the school year. Students will be withdrawn if not in full-time residence as indicated on the authorization form.
